Popis: The patient is a 14-year-old black female who presents with a 1-year history of abnormal uterine bleeding. She started her menses at age 13. Her cycles range from 2 to 8 weeks in length. The periods last from 3 day to 3 weeks with occasional very heavy bleeding. She is not able to do her own hygiene as she cannot understand what to do with her pads. She takes them out and does not put a new one in. She lives with her father mother and two brothers. She goes to school with special ed classes. She has a complicated medical history with the following issues: Diabetes mellitus type1 grand mal seizures chronic hypertension autism mental retardation gastro-esophageal reflux congenital hiatal hernia. Her medications include: Insulin Glucophage Propulsid Prilosec and Lotensin. (excerpt)
