A LTHOUGH IT IS NOT always easy to answer the question “What is failing in acute respiratory failure?” it seems clear that respiratory muscle fatigue alone or in association with a dysfunction of any other component of the respiratory system (the respiratory centers in the CNS, the neuromuscular connections, the chest wall, or the lungs) can result in acute respiratory failure. This review focuses on five aspects of respiratory muscle function: the definition of respiratory muscle fatigue, the factors predisposing to the occurrence of this fatigue, the methods to assess respiratory muscle fatigue in the intensive care unit, the role of respiratory muscle fatigue in some clinical situations of acute respiratory failure, and finally, the therapeutic approach to respiratory muscle fatigue. |
