Popis: | Effective treatment of vascular injuries requests rapid detection and treatment of haemorrhage or ischemia. The most important parameters of success are: Awareness of potential vascular injury, careful physical examination, ample but time-efficient diagnosis, good cooperation between vascular surgery and interventional treatment, careful observation after surgery (and further treatment if necessary). Critical loss of time can be reduced considerably via: well-established clinical pathways, expert interpretation of spiral CT, early involvement of vascular surgeons and interventional experts, and on-site treatment if optimum technical equipment is available - if not, urgent transferal to a specialist who must be previously informed in detail is required. Specific vascular surgical treatment is required almost only with injuries of limbs and the aorta. Haemorrhage in the torso area can mostly be treated in a conventional way. Autologous vascular construction as well as interventional treatment with covered stents and coils have widened the field of treatment options considerably. Ischemia of the limbs lasting for more than 6 h, as well as failure to treat compartment syndrome, impair prognosis considerably. |
