Estrogens and hypertension

Popis: Effects of discontinuing estrogens on blood pressure exchangeable sodium and the renin-aldosterone system were studied in 44 patients who gave the history of the development of hypertension after starting an estrogenic compound. 13 of 27 patients on conjugated estrogens (CE) 3 of 3 on diethylstilbestrol (DES) and 8 of 14 on an oral contraceptive (OC) became normotensive on the average of 3.6 months after the estrogen was discontinued. 7 of 8 became hypertensive again on a repeat course of estrogens in 3 to 6 months. The average changes that had occurred after 1 month off the estrogenic compounds in the 44 patients (combined CE DES and OC groups) were as follows: sodium (Na) decreased -59.2 meq; renin substrate decreased -64% but was still above normal limits in 79% after 2-3 months of the estrogens; renin activity decreased -25% and -17% respectively under normal Na and low Na conditions; urinary aldosterone decreased -18% on normal Na conditions. These changes were significant (p < .05). Plasma aldosterone remained unchanged or increased slightly after the estrogens were discontinued. 13 patients elected to restart an estrogen to control their menopausal symptoms after conclusion of the above observations. All patients were kept within normal limits without side effects on combined estrogen with spironolactone-diuretic administration.
