Due to sympathetic activation heart failure patients generally develop an elevated heart rate, which further deteriorates pathological changes and symptoms in a circulus vitiosus. Interruption of this circle is an important mechanism of beta-blocker therapy in heart failure. However, there may be several contraindications to beta-blockers like asthma bronchiale. For these patients so far no reasonable alternative medication is available, particularly because verapamil should not be given due to its marked negative inotropic effect. We present a patient with severe myasthenia gravis, dilated cardiomyopathy and sinus tachycardia, who was markedly improved by the selective If inhibitor ivabradine. Case report |
