Popis: | The peritoneal microcirculation provides the framework for the physiological interactions between the systemic vasculature and the peritoneal cavity. In peritoneal dialysis, these dynamic interactions are of paramount importance in maintaining effective dialysis. The peritoneal microcirculation participates in numerous physiologic functions including solute transfer and exchange, regulation of fluid dynamics and ultrafiltration, delivery of nutrients and hormones, delivery of leukocytes to areas of inflammation, gas exchange, and distribution of drugs. Knowledge of the dynamics of the peritoneal microcirculation in relation to peritoneal dialysis provides a basis for the rational use of drugs and other manipulations to increase the efficiency of peritoneal dialysis. In addition, studies of the peritoneal microcirculation provide investigators a unique window through which many basic microcirculatory processes may be explored. The focus of this chapter will be to review the available information regarding peritoneal micro-circulatory function in relation to peritoneal dialysis by examining the following areas: (1) the functional anatomy of the peritoneal microcirculation, (2) the physiologic factors modifying the peritoneal microcirculation and (3) the role of the peritoneal microcirculation in inflammation. |
