Popis: | We present the case of a patient who had tolerated angiotensin converting-enzyme inhibitor (ACEI) therapy for several months without adverse effects until he suddenly developed life-threatening angioedema of the upper airway, necessitating tracheal intubation. The patient had undergone heart transplantation two years prior to the episode of angioedema. Besides the condition of long-term ACEI therapy we identified three possible triggers for the development of angioedema: consumption of alcoholic beverages, acute pneumonia and long-term use of cyclosporin A. The patient responded slowly to conventional pharmacologic therapy, could be extubated after seven days, and left the hospital after 34 days. |
