The CBF evaluation with ECD SPECT allows a sensitive identification of clinically inapparent cerebral perfusion disturbances during the balloon occlusion test (BOT). A symmetric perfusion pattern or only slight defects with an asymmetry less than 9% and no neurological symptoms during the BOT means only a low risk for stroke after sacrifice of the ICA. However, a hemispheric or territorial perfusion defect with an asymmetry of greater than 9% or the occurence of neurological symptoms during the BOT is associated with a high risk for a stroke after permanent occlusion or resection of the ICA. |
