| Popis: | Given that malignant tumours of the pancreas are associated with a poor prognosis, the management of the majority of patients is focused on achieving optimal palliation of symptoms and maintaining quality of life. The diagnosis, staging and treatment of patients with malignancy of the pancreas and periampullary region remains a clinical challenge because of its retroperitoneal location, its complex relationship with blood vessels and viscera and the propensity for occult intraabdominal metastases at the time of presentation (1). The aim is therefore to select reliably patients for whom attempts at curative pancreatic resection are appropriate, while identifying those with factors which contraindicate surgery. Modern radiological imaging techniques may be misleading in the assessment of pancreatic malignancy; this is reflected by non-therapeutic laparotomy rates of 12 to 39% (2–5). The associated physical and psychological morbidity, and the cost-inefficiency of this strategy has led to increasing recognition that “laparotomy and biopsy” is an unacceptable primary method of assessing such patients (6). |
