THE EVALUATION OF CLINICAL ACTIVITY AND EFFICIENCY OF TREATMENT OF THE INFANTILE HEMANGIOMA WITH THE HELP OF ULTRASOUND SCANING

Popis: | The aim of the investigation is to determine the ultrasonographic (US) features of the infantile hemangioma (IH) depending on the clinical stage in case of natural course and during treatment. 76 children with clinical signs of soft tissues IHs aged from 1 month up to 1.,5 years were enrolled. US examinations were performed with linear transducer 12-3 MHz in regimen of gray-scale and color duplex sonography (CDS). Patients were divided into three clinical groups: (1) patients with systemic propranolol treatment (n=28); (2) patients with topical propranolol ointment treatment (n=14); (3) control group (n=20), which were only observed. In gray-scale regimen the IH’s echogenicity and size were determined. According the results of CDS, four stages of IH’s activity were marked out: prodromal, proliferation, stabilization regression. The most aggressive growing occurs in proliferative stage, it’s the objective hallmark of systemic therapy prescribe. The duration of proliferative stage in control group was 25±4,.8 weeks, Iin case of systemic ẞ-blockers treatment its duration was reduced to 2±1.,4 weeks. Topical treatment was used in children with superficial lesions when definition of the stage is difficult. The main aim of US examination in this group is measuring of lesion’s depth. A conclusion is made that US in CDS regimen is the objective method of the evaluation of IH’s clinical stages and its treatment effectiveness. The sign of systemic IH’s treatment efficiency is reducing the length of proliferative stage. Lack of US examination is limiting the area of visualization, and difficulties in diagnosis of superficial flat lesions. |
