Optimizing of preoperative preparation and anesthesia care in newborns with necrotizing enterocolitis

Popis: | The article presents the results of a study of 128 patients to assess the clinical course of preoperative preparation, anesthetic technique, the study of central hemodynamics, mathematical analysis of heart rhythm in newborns operated on necrotizing enterocolitis (NEC). Depending on the method of anesthesia, patients were divided into 4 groups. Studies were carried out on 5-stages of anesthesia: after premedication, induction of anesthesia, the beginning of the operation, the time of surgery and traumatic awakening. Assessment of hemodynamic parameters was provided: heart rate, blood pressure. Study on the application of reosorbilackt showed that the use of this drug in the hypovolemic state in newborn helped to improve as a central hemodynamics and microcirculation. Multi-agent anesthesia with an inhalation anesthetic sevoflurane is characterized by fast and easy induction, leads to decrease the need in muscle relaxants by 20%. The use of multi-agent anesthesia with GHB in combination with fentanyl is the best method for emergency surgical interventions with hypodynamic type of circulation. |
