Popis: | Over the past 80 years, there have been numerous descriptions of techniques to block the brachial plexus at the levels of roots, trunks, divisions, cords, and peripheral nerves. This article emphasizes the multicompartmental nature of the brachial plexus at each of these levels. Whatever site of block is chosen, the local anesthetic should be deposited in a manner that ensures some drug distribution to each compartment. In general, this means using a small mobile needle in a fanning manner versus the concept of an immobile needle depositing a large volume of anesthetic in a single reservoir. Paresthesia will often occur with all techniques and we must recognize this as reality, interpret their meaning as generally being helpful, not harmful, and seek to minimize any discomfort or possible harm to the patient. A “successful” nerve block has many possible descriptors, but every anesthesiologist appreciates the usefulness of effective, rapid onset, brachial plexus anesthesia. |
