Popis: | The authors review the poor reinvasion of massive cortical allografts. To promote revascularisation they tested the efficacy of cortical perforations and of an osteo-inductive material, ie powdered decalcified bone. The experiment consisted of performing in adult sheep a 5-cm resection of the tibial diaphyhsis bridged by a locked nail and its replacement by a cement spacer or by nothing at all in a first group, by a simple allograft in a second group, by a perforated allograft in a third and by an allograft surrounded by an osteo-inductive cuff in the fourth. Radiographic study at sacrifice showed no significant difference between the 3 groups of allografts, even though incorporation of the perforated allografts is better than that of nonperforated allografts. Though the osteoinductive cuff certainly induces new bone, it does not decrease the percentage of pseudarthroses. |
