Disseminated African Histoplasmosis in a White Heterosexual Male Patient with the Acquired Immune Deficiency Syndrome/Disseminierte Afrikanische Histoplasmose bei einem weißen Heterosexuellen mit AIDS

Popis: One of the 1st known cases of disseminated histoplasmosis as a result of histoplasma duboisii was in a patient with AIDS. This patient a 38 year old caucasian was diagnosed in 1986 and had lived in Zaire from 1972 until 1978. There have been about 20 cases of disseminated histoplasmosis diagnosed in patients with AIDS since 1979. A Giemsa-stained impression of a cervical lymph node biopsy indicated many large yeast cells. After incubation numerous small white colonies developed. Similar colonies developed from a culture of sternal bone marrow. The cervical lymph node had lost its normal features and contained numerous large round to oval shaped yeast cells. This patient was successfully treated with amphotericin B followed by oral ketoconazole. H duboisii is different from H capsulatum in its geographical distribution clinical symptoms and its yeast phase morphology. H capsulatum is normally found in the Mississippi and Missouri valleys and in South and East Africa. H duboisii results in a rare disease found in Central and West Africa. H capsulatum has been known to cause opportunistic infections in AIDS patients but there have been no reports of disseminated African histoplasmosis in these cases. With the prevalence of AIDS infection in Central Africa we would expect to see this association more frequently.
