The use of internal fixation in performing triple arthrodesis in adults

Popis: 25 adult patients (29 feet) underwent triple arthrodeses utilizing internal fixation, consisting of cancellous bone screws and power driven titanium staples. 23 females and 2 males with an average age of 43 years (range 15–70) were followed an average of 4.0 years (2.1–6.3 years). There were 20 planovalgus and 9 cavovarus foot deformities. The average length of postoperative immobilization was 11.4 weeks (range 6–18 weeks). Pain relief was rated excellent in 12 feet (41%), good in 7 feet (24%), fair in 4 feet (14%) and poor in 6 feet (21%). However, of the 10 feet with fair and poor results only 3 had residual pain as a result of the triple arthrodeses. 18 patients (72%) had an improvement in their walking score, and the walking score worsened in only 2 patients (8%). Ankle pain was responsible for most of the fair and poor pain and walking scores. Six of the 9 patients with progressive ankle joint degeneration had preoperative evidence of talar instability (talar tilt greater than 5°) on standing radiographs. Complications occurred in 5 patients (17%), 3 of which were non-unions requiring revision surgery. Therefore it appears that these methods of internal fixation may be useful in obtaining a triple arthrodesis. However they do not improve the overall results of triple arthrodesis in the type of adult patients we studied. The final result appears to be affected by the patient's disease, the foot deformity and the degree of ankle joint involvement, more than the technique of fixation utilized in the operation.
