POLYOSTOTIC FIBROUS DYSPLASIA DIFFERENTIATED IN INTERMEDIATE-GRADE SPINDLE CELL SARCOMA: A CASE REPORT

Popis: A 53-year-old female patient presented with complaint of volume increase in the face with 5 months’ of evolution. Physical examination showed a right hemifacial tumor, spontaneous pain, dystopia, amaurosis, right nasal obstruction, and multinodular hardened lesions on the right upper alveolar ridge. The tomographic examination revealed hyperdense lesions in frontal bone enveloping the ethmoid bilaterally, maxillary sinus, and nasal cavity, in addition to the other long bones, which led to the diagnosis of polyostotic fibrous dysplasia. No other endocrinologic finding suggested a syndromic variant, and the rapid and progressive growth led the team to investigate the malignancy. After an angiotomographic study, intense periosteum activity and central vascularization by the external carotid artery were observed. Biopsy was chosen through the core biopsy method, in which the immunohistochemistry confirmed the differentiation in intermediate-grade spindle cell sarcoma. The proposed treatment was chemotherapy and radiotherapy, evolving to death within 3 months’ of the disease.
