Since the first application of an anaesthetic, anaesthesia providers have been looking for a parameter that reliably predicts the depth of anaesthesia. Over the past decades the mechanisms of action of the various groups of anaesthetic agents have been elucidated. The notion that different anaesthetic agents propagate the anaesthetic state through different mechanisms and receptor systems has lead to the believe that no such thing as a single anaesthetic state exists but that anaesthesia may embody different levels of sedation/unconsciousness, analgesia, autonomic nervous system stability and muscle relaxation. Consequently, it may be unrealistic to search for a single parameter that reliably reflects the depth of anaesthesia. Nevertheless, over the past decades the search for such a parameter has been intense. Since 5 years now the bispectral index scale (BIS) monitor is the one and only clinical available device that has culminated from this search and became available for clinical practice. This manuscript describes the current state of knowledge on the bispectral index scale monitor and its present place in clinical practice. |
