Late orbital recurrence after enucleation for malignarit melanoma of the choroid

Popis: A patient underwent perforating diathermy and aspiration of subretinal fluid for a non-rhegmatogenous retinal detachment. Tumors cells were found in the sub-retinal fluid and the eye was enucleated four days after the unsuccessful reattachment operation. Pathologic examination showed no evidence of scleral invasion or extraocular extension of tumor. Twenty-nine years later, an orbital exenteration was performed for recurrent malignant melanoma, the longest recorded interval for a case showing no evidence of extraocular tumor extension at time of surgery.Patients undergoing ocular surgery invblving scleral penetration who are subsequently found to harbor an unsuspected malignant melanoma are likely best managed by prompt orbital exenteration. Surgical manipulation over the tumor has the potential of expressing tumor cells into the orbit via emissaria. The possibility of late recurrence of orbital malignant melanoma in patients subjected to such procedures should be considered.
