Systemic steroids (glucocorticoids) are used ubiquitously in the hospital for a variety of indications. Although they have clear and wide-ranging benefits, they are also a major contributor to severe hyperglycemia. In patients with and without preexisting diabetes, steroids can exacerbate glycemic control and can make glucose management in the hospital challenging. Variability in steroid type, dose, and dosing schedules poses additional challenges to proactive adjustments in glycemic management strategies that promote safe blood glucose (BG) control and avoidance of hypoglycemia. Moreover, many patients receiving steroids have comorbid conditions or are receiving other medications that can affect glucose homeostasis. An understanding of the pathophysiology and pattern of steroid-associated hyperglycemia is necessary to guide management of hospitalized patients on steroids. |
