W., aged 34, a Cherokee Indian, was first seen at the Vanderbilt Clinic April 4, 1946, complaining of a vesicular dermatitis which has appeared each August for the past twenty years on the lower half of the legs. The lesions gradually disappear by winter, leaving pigmentation and infiltration. The patient has never noticed lesions on any other part of the body, except that the toe nails have never been normal. His general health has been good except for recent symptoms of a gastric ulcer. Examination shows the legs to be covered with a mosaic pattern of active and inactive lesions from the midline between the knee and ankle down to the ankle. The active
