Popis: Leptospirosis. Clinical syndromes Leptospirosis is a zoonotic disease that is widespread in tropical regions and is less common in temperate zones. It is transmitted by the spirochete Leptospira interrogans. Leptospira are excreted in the urine of animals that act as a reservoir; rodents in the main, although it can affect other mammals as well. It is transmitted to humans through contact with contaminated water. It affects groups at risk who are exposed to animal reservoirs or contaminated environments, such as rice farmers, slaughterhouse or sewage workers. It is commonly diagnosed in people who present with a fever after international travel where they have taken part in water activities. The form of presentation is very variable, from self-limiting febrile diseases often confused with common conditions such as influenza, to diseases with multiorgan involvement such as Weil's syndrome, characterised by renal failure, jaundice and haemorrhage. In this update we describe the most relevant clinical aspects for a suspected diagnosis, as well as the key aspects for definitive diagnosis and appropriate treatment.
