Fifty-seven consecutive IVF couples were investigated to determine whether the induced acrosome reaction could be used as an index of fertilization potential. Diagnostic test characteristics were used to validate the effectiveness of the test. Both follicular fluid and the calcium ionophore A23187 increased acrosome reaction. The follicular fluid-induced acrosome reaction showed a strong correlation with fertilization of oocytes and strict criteria sperm morphology. A cut-off value of 15 percent acrosome reaction gave the best predictive value for fertilization. The receiver operating characteristic (ROC) curve for greater than 33 percent fertilization indicated that there was a 79 percent chance of predicting fertilization outcome with the results of the acrosome reaction test. When sperm samples with low acrosome reaction and subsequent poor fertilization rate were incubated overnight and then treated with follicular fluid, the acrosome reaction increased two-fold. Because a significant follicular fluid-induced acrosome reaction was associated with a good fertilization rate, these results suggest that in IVF couples with poor fertilization resulting from sperm that have a low acrosome reaction, overnight incubation of sperm in a culture medium might improve the rate. |
