Popis: Cysts in the paranasal sinuses are common, but a primary cyst in the temporal bone is a rare occurrence. A 64-year-old female suffered from migraines for fifteen years, in addition to left hearing loss with tinnitus for three months after a head injury as a result of her husband's violence. The tympanic membrane was normal and an audiogram showed left combined hearing loss. X-ray examination revealed two round regions of bone destruction of the squamous temporal bone, which appeared as two spacies occupying the region above the pyramidal bone. CT revealed two low density areas, two cholesterol cysts with osseous obstruction of the aditus. MRI revealed that the two round areas were distinct from the surrounding tissue. MRangiography revealed that the left sinus sigmoideus was slightly compressed. Two cysts which occupied wide areas of the left temporal bone were removed surgically. Exploratory tympanotomy revealed, disruption of the ossicular chain. Restoration of the ossicular chain was performed. Post-operatively, the patient's hearing was improved and her tinnitus was cured.
