Popis: Discussion: Pathophysiologic mechanisms proposed for Broken Heart Syndrome include multivessel coronary spasm, microvascular coronary spasm, catecholamine-mediated cardiotoxicity, acute myocarditis, transient obstruction to LV outflow, and the rupture of a non obstructive plaque followed by spontaneous thrombolysis. These two pathophysiologic mechanisms microvascular dysfunction and catecholamine-mediated myocardial stunning may be correlated since a catecholamine-mediated endothelial injury might be the cause of the microvascular dysfuction seen in patients with ABS. Fortunately, the fatality rate from Stress Cardiomyopathy is less than that of acute myocardial infarction (about 4%). The marked left ventricular dysfunction and the peculiar shape usually disappear within several weeks, but it is possible recurrence.
