Assessment of Obesity and Cardiovascular Risk in South Asians

Popis: In 2010, coronary disease was officially recognized as the number one cause of death in India. However, it has long been known that the prevalence of cardiovascular disease in South Asians is significantly higher than in other ethnic groups. More than 60 % of the world’s coronary disease burden resides in South Asia, and the first coronary event occurs, on average, over 10 years earlier in South Asians than in other ethnicities. This higher coronary event rate is attributed to both the earlier onset and high incidence of several coronary risk factors, such as obesity/metabolic syndrome, dyslipidemia, and insulin resistance/diabetes, coupled with a sedentary lifestyle. Despite the abundance of traditional risk factors, South Asian cardiovascular mortality is grossly underestimated by traditional risk factor modeling (e.g., Framingham, QRISK, Reynolds, etc.). Nontraditional risk factors, which include more advanced lipid and inflammatory biomarkers, adjusted body mass index (BMI) cutoffs, and even progenitor cell assessment, are independently predictive of risk in the South Asian population and may provide enhanced risk assessment beyond traditional models. This review details the magnitude of the coronary disease burden in the South Asian population, the traditional risk factors that may drive this South Asian epidemic, the limitations of these same traditional risk factors in assessing this risk, and the potential of novel inflammatory and lipid biomarkers to better capture this risk in the South Asian population.
