Popis: | Viral pneumonia, frequently mild and requiring only supportive care, when diffuse and fulminating, can be a life-threatening illness. Acute respiratory failure, with ever-worsening hypoxemia can be very resistant to conventional intensive respiratory management This presentation recounts two cases of diffuse interstitial pneumonitis with severe respiratory failure. Because excessive lung water was thought to be an important pathophysiologic factor, these patients were treated with a program of intensive dehydration, and dramatically responded in a favorable manner. Such therapy has been used in other varieties (post-traumatic pulmonary insufficiency or shock-lung) of the adult respiratory distress syndrome but has not been reported in this circumstance. Details of management and the rationale thereof are presented. |
