BLOOD SUPPLY OF THE LOWER EXTREMITY IN PATIENTS WITH ISCHEMIC FORM OF DIAETIC FOOT SYNDROME

Popis: The aim of the study was to compare the features of the blood supply of the lower extremity in patients with atherosclerosis and diabetes mellitus. Material and methods. The results of examination and surgical treatment of 22 patients with diabetes mellitus and lesion of the main arteries of the lower extremity are adduced in the article. According to the data of ultrasound dopplerography and artiography, the levels of arterial lesions were distinguished: occlusion of the aorta-femoral segment, femoral-popliteal segment, popliteal-shin-foot segment. Reconstructive procedure with restoration of blood supply of the lower extremity on arteries was performed in 67 patients. Results. Hemodynamics peculiarities were described apart and in comparison with the identical groups of patients with obliterating atherosclerosis before and after surgery. It has been established that manifestations of foot ischemia in patients with diabetes mellitus are more severe than in patients with atherosclerosis. When performing surgical interventions, it is advisable to perform distal reconstructions taking into account the multilevel lesion. Conclusions. With the same level of arterial disease, the severity of foot ischemia in patients with diabetes is more severe than in patients with atherosclerosis. Taking into account the multilevel lesion, patients with diabetes mellitus are candidates for distal reconstruction on arteries of femoral-popliteal segment.
