Popis:	We present the case of a L4 fracture (Magerl A 2.2) in an elderly patient that was at first conservatively treated. After 6 months a posterior L3–L5 stabilization was performed. A pseudoarthrosis developed over time because of the coronal split of the vertebral body. The final treatment was an extension of the posterior arthrodesis to the sacrum and an anterior somatectomy of L4 with the implant of an expandable cage. We present our considerations about the surgical options.
