A CASE OF GASTRIC CANCER PRESENTED WITH RUPTURE OF METASTATIC LYMPH NODE

Popis: | A 68-year-old man was admitted to the hospital because of sudden sever abdominal pain. Abdominal US and CT revealed an abdominal tumor and peritoneal bleeding. Because of increasing pain, bleeding and anemia, an emergency laparotomy was carried out on the next day. A12⁄12×10cm big tumor, involving the stomach, duodenum, pancreas head and transverse colon, was partially ruptured and about 1000cc of blood pool was noted. Pancreatoduodenectomy with a partial resection of the transverse colon was performed. The tomor was partially solid and the others were polycystic with varied appearance. Additionally, there was a 4×3cm Borrmann II type tumor in the gastric antrum. Histological examination revealed that both the tumor and gastic lesion were well differentiated adenocarcinoma with infiltration to the duodenum, pancreas and transverse colon. The tumor might be development of extragastric growth of the gastric cancer. However, the further histological investigation detected that the tumor contained some remnant structures of the lymph node and the other metastatic lymph nodes contained the same appearance as the tumor. So the definitive diagnosis of the tumor was development of the metastatic lymph node of the gastric cancer. This case appears so rare. |
