Non-Surgical Management of Meningiomas Involving the Cavernous Sinus

Popis: Aims: Meningiomas involving the cavernous sinus usually confer surgical non-respectability. This review evaluates the Prince of Wales Cancer Centre’s experience with treatment of this type of tumor. Methods: From 1990 to December 2004, 283 patients were referred to the Department of Radiation Oncology of whom 147 patients were treated with radiotherapy. Eligibility was involvement of the cavernous sinus, not necessarily the origin. 91 patients presented for initial treatment; 56 had onset of progressive disease. Treatment comprised conventional radiotherapy, stereotactic radiosurgery (SRS), and stereotactic fractionated (SRT) approaches. Decision of SRS versus SRT was based on tumor volume and/or proximity to optic chiasm. Results: Most patients had clinical features, or imaging, demonstrating progressive disease. Age range was 7–82 years with 110 (75%) females. Treatment approaches were fractionated for 80 patients (50.4 Gy/28 fractions); the median dose for radiosurgery in 56 patients was 14 Gy; 10 patients had conventional radiotherapy.
