Acid-base disorders are very frequent. Metabolic alkalosis and respiratory alkalosis are primary disorders. Both metabolic and respiratory alkalosis produces compensatory changes on contrary. But there are also mixed disorders. There are many causes for them. In metabolic alkalosis the most frequent causes are gastrointestinal (vomiting or nasogastric suction) or renal (diuretic) loss of H+. Treatment of these disorders will be different if the causes are chloride sensitive or chloride resistant. Causes of respiratory alkalosis are hypoxemia, respiratory center stimulating or excessive mechanical ventilation. This disorder presents more clinical symptoms than metabolic alkalosis because CO2 goes through the hematoencephalic barrier more quickly. Its treatment is CO2 reinhalation or sedation and mechanical ventilation. |
