Spine trauma induces a profound immunological response which renders the spinal cord, as well as remote organs susceptible to a so-called “2nd hit” injury. The adequate timing and modality of spine fixation are therefore crucial for avoiding a deleterious outcome. A detailed understanding of the basic pathophysiological changes is a prerequisite for the design of optimized treatment strategies in patients with major trauma and associated spine injuries. The present overview will summarize the current knowledge of the cellular and molecular changes that occur after spinal trauma and evaluate new therapeutic targets and strategies. The new concept of “spine damage control,” which bases the surgical decision-making on the patient’s immunological state, will be discussed. |
