Hämodiafiltration – Vorteile müssen sich noch in Studien erweisen

Popis: Haemodiafiltration provides superior middle molecule clearance. Online haemodiafiltration is particularly effective because a large volume of substitution fluid can be produced at minimal costs. However, the intermittent nature of haemodiafiltration treatment limits the removal of middle molecules because the intercompartimental transfer of substances within the body is the rate-limiting step of removal. In addition, protein-bound uraemic toxins will not enter the dialysate compartment, even if additional convective transport is applied in haemodiafiltration. The beneficial effects of haemodiafiltration may be difficult to prove: Patient populations with a high mortality rate may have other overwhelming reasons to die prematurely. On the other hand, it is very difficult to demonstrate a survival benefit in patients with a good long-term prognosis, as it may take years or decades to see the consequences. Prospective randomized trials are currently under way to test the hypothesis that increased removal of middle molecules provides superior survival for haemodialysis patients.
