Short TML occurred during percutaneous dilatational tracheostomy for long term invasive ventilation. The tube’s tip went into the mediastinum. The lesion was detected two days after tracheostomy and intermediate attacks of increased air way resistance. Intraluminal repair was performed with a 2/0 Vicryl thread with UCLX needle (Ethicon, Germany) using a 14 mm tracheoscope and the optical needle holder (KARL STORZ, Endoscopes, Germany)
