A Randomized Clinical Trial Evaluating Dietary Counselling With or Without Enteral Supplementation on Nutritional and Functional Outcome Parameters In Malnourished Hiv-infected Outpatients

Popis: | In HIV-infected individuals, malnutrition and wasting are common complications, which may be associated with diminished functional and cognitive capacity. Although nutritional therapy has been advised in such patients, few data are available concerning the effects of this intervention. We performed a 6-week randomized clinical trial which compared dietary counselling alone (control) versus dietary counselling plus enteral supplement (Lipisorb) in 118 male HIV-infected outpatients. The inclusion criteria were: age ≥ 18 yr; CD4+ cell count 3 ; and 10% body weight in the previous 6 months. Each patient was instructed to consume a diet which contained 960 calories greater than estimated total energy expenditure. Nutritional outcomes were: skinfold measurements, fat free mass and total body water (Bioelectrical Impedance Analysis). Functional outcomes were: grip strength (Jamar dynamometer), cognitive function (Buschke Selective Reminding test) and Quality of Life (instrument designed for this study). Ninety-nine (99) men completed the study. No differences in skinfold measurements or Quality of Life parameters were observed between the treatment groups. Compared with the control group, the Lipisorb group experienced significantly fewer decreases in total body water (p =.04). Fat free mass (p =0.07) and grip strength (p = 0.07) were increased in the Lipisorb group, although the differences did not reach statistical significance. Significant improvements in short-term memory (p = 0.03) and long-term memory storage and retrieval (p = 0.05) were observed in the Lipisorb group. In malnourished HIV-infected outpatients, dietary counselling with enteral supplementation is associated with improved cognitive function. In addition, the effects of this intervention on nutritional parameters and muscle function are promising. |
