Popis: | Background: Seasonal trends in adult pregnancy conception rates have been established in many studies. The objective of this study is to determine if a seasonal trend exists in conception of adolescent pregnancies compared to pregnancies in an adult population. Methodology: A retrospective chart review of all adolescent pregnancies over a 5-year period (N5838) was undertaken at a single tertiary care hospital, offering the only obstetrical services in the region. A similar review of a random sample of 838 of the 13,554 adult pregnancies, during the same period of time, was performed. Pregnancies ending in spontaneous abortions, elective terminations, ectopic pregnancies, as well as obstetrical deliveries were included. Conception date was estimated by either last menstrual period or earliest ultrasound available. A chi-square goodness of fit test was used to compare the proportion of pregnancies conceived in each calendar month. The Canadian city of interest has a one-site (and tertiary level) model of care where any outpatient or inpatient pregnancy-related presentations are recorded and tracked. The only pregnancies that would not be included would be those spontaneously miscarried without the need for medical attention or those delivered at home who never required an assessment at the medical centre. Results: There was a significantly different monthly trend seen in adolescent pregnancy conceptions when compared with the adult group (c524.38, df 5 11, p50.0112). Notably, the adolescent group was observed to have a unique peak in the number of pregnancies conceived in the month of March (10.5% of pregnancies conceived compared to 7.3% in adults). In addition, 8.5% of conceptions in adolescents occurred in the month of December compared to 10.4% in adults; the December peak evident in the adult group of this study and absent from the adolescent group has been previously documented in literature pertaining to adult conception trends. Conclusions: Compared to adult women, adolescents have an increased likelihood of conceiving in March and are less likely to get pregnant in the month of December. There are several plausible explanations for the modest but real differences identified in this study such as trends in fecundity/fertility or social/ school events that lead to increased sexual activity. Regardless, peaks in conception indicate times when a greater need for health care services exists, and when preventative education can be most effective. |
