Educational discharge training forms to patients with home mechanical ventilation

Popis: Among chronic respiratory patients discharged at home, patients under mechanical ventilation (MV) are the more critical subgroup. The mandatory multidisciplinary care and assistance for them is particularly difficult and complex and pre-discharge educational training with specific items is currently recommended, but it does not exist a scientific consensus of instruments, training form, sheets to be used for this. Aims of this article are identify main educational pre-discharge needs and to create dedicated training forms. The study was conducted in two respiratory units trough three stages: (1) identification of needs and forms preparation by a primary team work (PTW), (2) training forms discussion by a questionnaire submitted to an Expert Staff (ES), (3) training forms final approval. Stage 1: Needs of patient were synthesized by PTW in 51 items resumed in six training forms: Bronchoaspiration, Tracheostomy tube management, Respiratory Problems, Use of Cough Assist Machine, Oxygen saturation measurement and Ventilator Use. Stage 2: ES was composed by 47 health operators, the average score of questionnaire answers is 8.46 ± 0.22 (best 10, worse 0). Twenty-five requests of forms changes were suggested. Stage 3: 21 items (45%) were changed. Our study proposes 51 items with 6 discharging educational forms for make an easy and uniform educational training for patient using MV at home. Future studies should be addressed to validate use of these forms on a larger patient population.
