The prevalence of colorectal carcinoma during pregnancy is low but is associated with significant fetal and maternal morbidity and mortality. This chapter characterizes diagnostic features, evaluation techniques, and therapeutic strategies for patients who present with colorectal cancer during pregnancy. Pregnant patients frequently present with advanced stage of disease secondary to delayed diagnosis, which results in poor prognosis. An in-depth understanding of the current diagnostic armamentarium can improve diagnostic potential while limiting possible risk to the fetus. In addition, this chapter offers surgical techniques germane to pelvic operations in the presence of a gravid uterus and explores the current pharmacological options available to these patients. The combination of a life-threatening illness during pregnancy and the unfavorable prognosis of colorectal cancer raise many ethical, psychological, and emotional concerns in the therapeutic decision-making process. A holistic approach with a multidisciplinary team consisting of the obstetrician, surgeon, oncologist, maternal-fetal medicine specialist, and social worker should be employed in the management of these patients. |
