Kasuistik einer Leiomyomatose pelviner Lymphknoten unter Östrogensubstitution

Popis: A 68-year old, para 1, woman receiving hormone replacement therapy was admitted because of a histologically proven endometrial adenocarcinoma. Computed tomography (CT) scanning revealed enlarged mediastinal lymph nodes suspicious for lymph node metastases of the endometrial carcinoma. She underwent total abdominal hysterectomy with bilateral oophorectomy and pelvic lymphadenectomy. Histology only showed an endometrial adenocarcinoma FIGO IC, grading 2.The pelvic lymph nodes revealed only leiomyomatosis without evidence of metastatic endometrial carcinoma. Hormonal replacement therapy was discontinued. Six months postoperatively, the mediastinal lymph nodes had decreased considerably in size, as shown by a control CT scan. Nine cases of leiomyomatosis in lymph nodes have been reported in the literature to date. In these case the indications for surgery included uterine myomas and cervical or endometrial carcinomas. It is still unclear whether leiomyomatosis of lymph nodes and so-called benign metastasising leiomyomas are metastatic lesions.
