An audit of the management of spontaneous pneumothorax (SP) at a UK hospital

Popis: Introduction: Despite having national guidelines for the management of SP, there appears to be wide variation in clinical practice in UK hospitals. Aims and objectives: The audit aims to study the initial management of primary (PSP) and secondary (SSP) pneumothorax in a UK Hospital against the current BTS guideline. Methods: Data collection was made from patients9 records with a diagnosis of pneumothorax (PNX) over 4 years. Results: Sixty-eight patients were identified with SP and 56 being classified into PSP (n=31) and SSP (n=25). A large PNX was present in 23 patients with PSP and 60.8% of them (n=14) were managed initially by needle aspiration as per the guideline. Six others with small PSP were managed by varying procedures. Eleven (61.1%) of the 18 patients who received aspiration as initial treatment, required a chest drain subsequently. A large SP was present in 13 patients with SSP. Only 7 (53.8%) patients complied with the guideline by receiving chest drain and the other 6 were managed by needle aspiration. Small SP was present in 9 patients with SSP and 4 of them received chest drain, hence not complied with the guidelines. Overall, 62.5% of patients initially managed with needle aspiration (n=5) subsequently required chest drain. Eight (32%) patients were referred for surgery. Conclusion: There is suboptimal compliance to the BTS guideline in the management of SP. In addition, needle aspiration as the first line treatment has not shown to be effective in the majority of the patients with SP.
