520: Value of the natriuretic peptide proANP as a prognostic marker in pediatric heart transplantation

Popis: | used as adjunctive therapy twice weekly for 4 weeks. Results: There were no acute complications and none of the patients suffered any serious infections (one patient had an enterococcal UTI requiring antibiotic therapy). All patients were lymphopenic prior to Campath-1H, with absolute lymphocyte counts of 32-100 cells/ l. At 3 months post-Campath-1H, the hemoglobin and platelet counts for all patients remained normal. White blood cell counts ranged from 11004000 cells/ l with absolute lymphocyte counts of 100-500 cells/ l. By 3 months post-treatment, fractional shortening significantly improved from 28.6 3.9% to 45.3 4.2% (p 0.05). Two patients have had repeat endomyocardial biopsies, each with ISHLT scores of 1A. Maintenance immunosuppression consists of tacrolimus and mycophenolate mofetil. Conclusions: Campath-1H was used with predictable side effects (e.g. leukopenia), but without significant morbidity. Resistant rejection may be mediated independent of T cells, offering a potential explanation for Campth-1H’s utility. Long term follow-up and further study is warranted, but this experience demonstrates that Campath-1H can be effective for the treatment of resistant rejection in pediatric heart transplant recipients. |
