Improving standards of NIV - Simulation training

Popis: Introduction: Non-invasive ventilation (NIV) is now an established therapeutic intervention in hypercapnic respiratory failure. Hospitals require various members of the Multidisciplinary Team to initiate, manage and execute this service. However, junior doctors report lack of knowledge and confidence in NIV. Training is variable and the National COPD Audit highlighted the need for a structured programme. This and several recent clinical incidents involving patient safety and poor titration of NIV prompted the initiation of a quality improvement project. Method: Simulation based NIV course introduced in 2014. Consisting of a 30 minute lecture on theoretical aspects, 20 minute high-fidelity scenario with SimMan 3G model and in-depth structured debrief. Session covered all aspects involved in the BTS competency checklist. Participants decisions impacted on patient9s status. Pre- and post-test questionnaires using a 5 point Likert scale collected after closure of the session. Results: All 24 participants described improved confidence in the 4 areas, Wilcoxon matched-pairs signed rank test used to test significance. Conclusions: High fidelity Simulation training in NIV provides an effective and safe method to develop knowledge and skills. Simulations additionally allow practice and assessment of non-technical skills.
