A PRIMARY RESECTED CASE OF SYNCHRONOUS SQUAMOUS CARCINOMA OF THE THORACIC ESOPHAGUS AND HEPATOCELLULAR CARCINOMA

Popis: The simultaneous occurrence of esophageal and hepatic tumors is rare. The operative procedure and timing are controversial. Recently we experienced a case of such double cancer which was successfully cured by simultaneous resection. A 70-year-old man was pointed out of an esophageal abnormality by UGI at a periodic examination, and was admitted to the hospital with an esophageal carcinoma in the distal esophagus which was diagnosed by endoscopic examination. There were previous histories of hemothor-ax, bronchoectasia and liver cirrhosis (HCV). On admission abdominal ultrasonography and computed tomography revealed a hepatic tumor, 3cm in diameter, at the lateral segment (S3). His serum alpha-fetoprotein (AFP) level was elevated (37.6ng/ml). The patient was diagnosed as synchronous esophageal carcinoma and HCC. After the severity of his respiratory and liver functions was assessed, transthoracic subtotal esophagectomy and partial resection of the liver were primarily performed. Histopathologic examination confirmed the relative curative resection of the esophageal carcinoma and curative resection of HCC. The increased serum AFP level was normalized after the operation. No recurrent sign has been found as of 15 months after this surgical management.
