A CASE OF FIBROMA OF THE SPLEEN WITH AN ADVANCED GASTRIC CANCER

Popis: | Primary benign tumors of the spleen are rare. We report a case of splenic fibroma with an advanced gastric cancer which was indistinguishable from splenic metastasis. A 38-year-old woman was admitted to the hospital because of epigastric pain. UGI and GIF revealed an advanced cancer located from the upper body of stomach to the pylorus. Abdominal ultrasonography showed splenomegaly and abdominal CT showed a well-marginated low density mass in the spleen without contrast enhancement. Total gastrectomy with distal pancreatectomy, splenectomy and cholecystectomy was performed under a diagnosis of gastric cancer and splenic metastasis. The histological examination of the gastric lesion revealed poorly differentiated adenocarcinoma and the splenic tumor was fibroma. In a review of the Japanese literature, splenic metastasis of gastric cancer is characterized by internally heterogenous low density area on abdominal CT. This case which revealed a low density tumor with homogenous inner part must be suspected of primary splenic tumor as well. |
