We recently encountered a patient having a small protuberant lesion in the rectum, who has been affected with ulcerative colitis of the total colitis type for 22 years. Early cancer was suspected, and surgery was performed. Pathologically, invasion to the wall was pm, and lymph node metastasis was positive. The patient developed metachronous multiple metastases in the liver eight months after the surgery. The indication for surgery has been extended with the establishment of surgical therapy for ulcerative colitis. Even refractory case has recently tended to become a candidate for early surgery. Based on our experience with this case, earlier indication for operation should be considered because useless retardation can permit cancer change. The importance of surveillance of canceration for determining the timing of surgery is emphasized. |
