Isosorbide dinitrate (ISDN) is rapidly metabolized to isosorbide-5-mononitrate (IS-5-MN) and isosorbide-2-mononitrate (IS-2-MN). Needleman et al. [1] concluded from their findings in anesthetized rats that the two mononitrate metabolites of ISDN lack vasodilating properties. Later studies disproved these conclusions (e.g., [2]) and an antianginal efficacy of IS-5-MN was also shown in clinical studies [3, 4]. In most of our experiments on the pharmacologic properties of IS-5-MN we used conscious dogs. Although this is sometimes more difficult than experiments in anesthetized animals, in our opinion the results of such studies can be extrapolated more readily to clinically relevant situations. |
