Wedge resection of the external auditory canal: a temporary canal wall-down technique in cholesteatoma surgery

Popis: Objective. The surgical treatment for cholesteatoma is to create a dry and safe ear and to restore hearing when possible. The key factor is the dissection of the cholesteatoma matrix from the middle ear and mastoid, especially from obscure areas. Patients and methods. Two main types of approaches are available: canal wall-down and canal wall-up, each with their own advantages and disadvantages. At wedge resection of the superior-posterior wall has to be removed temporarily and replaced in one single operation. In this way, the advantages of both techniques are combined without the discomfort of a radical cavity and the chances for hearing restoration are good. During the period 1986–97, 38 patients were operated upon according to the described technique with a mean follow-up time of 36 months (range, 3–120 months) Results. At the last operation all ears were free from cholesteatoma without any residual cysts (7 × third look, 2 × fourth look). Two ears had to be converted to radical cavities because of a recurrent cholesteatoma. Conclusion. Wedge resection of the external auditory canal is an optimal approach to remove cholesteatoma from otherwise obscured areas. This approach is especially indicated in children.
