Traitement des fistules bronchopleurales par épiplooplastie en première intention

Objective: Bronchopleural fistula is an uncommon complication whose management remains a clinical challenge. We’d like to share our experience in the treatment of bronchopleural fistula by omentoplasty as first-line treatment (i.e., without open window thoracostomy). Methods: From April 2007 to September 2015, 12 patients with bronchopleural fistula who were operated on by omentoplasty were retrospectively included. The treatment was to perform (at the same time) the debridement of the pleural space by thoracotomy and then to take an omental flap by medial laparotomy. Reconstructive omentoplasty was performed before closing the thorax. Results: From our 12 patients (median age 62 years), 9 were successfully controlled. Two patients had recurrence of the fistula and were treated secondarily by an immediate open window thoracostomy. One patient died of septic shock. The median hospitalization time was 25 days. Conclusion: First-line omentoplasty is an effective method for the treatment of bronchopleural fistula and avoids open window thoracostomy in most cases. All patients without a history of abdominal surgery and whose septic condition is previously controlled can be eligible for this method. In case of failure, the open window thoracostomy must be carried out without delay.
