Popis: This report describes the clinical and surgical approach of a complex odontoma with periapical involvement implementing a new bioceramic cement for retrobturation. The patient presented for routine consultation with slight dental mobility in the anterosuperior region. On clinical, radiographic and computed tomography examinations, bone rarefaction was observed in the apical region of the elements 11 and 12, with density higher than bone and similar to teeth, 2.5 cm in diameter, external root resorption of the elements involved, and pulp necrosis of the 12. Conventional endodontic treatment was performed. In a second intervention, a surgical excision of the lesion was performed, showing a mass with clinical characteristics of complex odontoma. Element 12 was retrobturated with Aggregate Pentaoxide Mineral Bioceramic Cement (5 MO). Histopathological examination confirmed the diagnosis of complex odontoma. On follow-up there were no reports of symptoms and the clinical signs were satisfactory, with bone neoformation in the region. APOIO: FAPESP.
