Prosthetic graft infection after arterial reconstraction is surely one of the most fearful complications. In this paper three patients with such a complication are reported. Two patients with abdominal aortic aneurysm and one with Leriche syndrome underwent aortic reconstruction and involved retroperitoneal perigraft abscess early after the operation. All of them had signs of infection (fever, leucocytosis etc.). The diagnosis was established by abdominal CT scanning in all patients, though additional needle aspiration of perigraft fluid was done in one patient. Their abscesses might be caused by perioperative bleeding/graft occlusion, postoperative bleeding (hematoma) and colon necrosis/anastomotic leak, respectively. These three patients were successfully treated by infected graft excision with extraanatomic bypass. Total excisions of grafts with axillo-femoral bypasses in two and partial excision with femoro-femoral bypass in one were performed and now they are alive uneventfully. In order to save the patient from retroperitoneal perigraft abscess, making diagnosis as early as possible and consequent appropriate surgical treatment are essential. |
